NHS Grampian is seeking a motivated, compassionate and skilled Band 4 Healthcare Support Worker to join our Community Learning Disability Nursing Team in Moray. This is an exciting opportunity to support adults with learning disabilities to achieve better health outcomes and improved quality of life within their own communities.
The Role
As a Band 4 HCSW, you will work as an integral member of the multidisciplinary team under the supervision of registered Learning Disability Nurses. You will contribute to the assessment, planning, implementation and evaluation of personcentred care and support for individuals with learning disabilities and associated health needs.
The applicant should be educated to SVQ3 level or equivalent or relevant experience through short courses or equivalent experience.
The role involves community-based work, including home visits, clinics and liaison with families, carers and partner agencies.
The post holder should hold a current full driving license and have access to a car for work purposes.
