Overview
Social Worker - Medical Social Worker, Everett, Washington. Start date 06/01/2026 for a 13-week travel assignment. Weekly pay range $1,317–$1,386 per week. Shift: 4x8-hour days. Setting: Level II trauma center with two campuses.

Responsibilities
- Provide discharge planning and care coordination for adult and geriatric patients.
- Conduct biopsychosocial assessments and coordinate referrals to home health, rehabilitation, or long-term care as appropriate.
- Collaborate with an interdisciplinary team, including PT, pharmacy, transportation, interpretation services, and Social Services.
- Complete documentation in the EPIC charting system according to policy and standard workflows.
- Manage a caseload that typically ranges from 18–24 patients, with higher weekend volumes and a daily census around 25.

Requirements
- Licensure: Washington license to practice social work required; pending license accepted per client guidance.
- Education: Master’s degree in Social Work; MSW required. LMSW or LCSW with Master’s degree acceptable.
- Certifications: BLS required.
- Experience: Minimum 1 year of relevant experience (acute care, discharge planning, and care coordination); some client postings may require 2 years—verify with the client.
- Skills: EPIC charting proficiency; ability to perform biopsychosocial assessments and coordinate across services; ability to float across campuses as needed; Weekend and holiday availability per unit policy.
